NPHI

According to the National Partnership for Healthcare and Hospice Innovation (NPHI) website, over 5 million Americans are living with Alzheimer’s disease, the most common form of dementia, and that figure continues to grow each year.

In response to this growing need, NPHIAliviado Health, and the Center to Advance Palliative Care joined forces to create the Dementia Care Patient & Caregiver Resource Guide. This comprehensive guide is designed to support home care for individuals living with dementia and provide caregivers, including families, with the necessary skills and information to ensure patient safety, comfort, and well-being while reducing caregiver burden.
